Children's Health Insurance Program for Children under 19
Eligibility for the Children's Health Insurance Program (CHIP) is based on family size and gross monthly income. Children covered by an employer's health insurance plan in the past 3 months are not eligible for the Children's Health Insurance Program unless you've lost your job or your employer has ended health insurance coverage for your child. Federal law prohibits the Children's Health Insurance Program (CHIP) from providing coverage to children who are eligible for no-cost Medicaid. As part of the enrollment process, your child's application will be screened for Medicaid eligibility.Children's Health Insurance Program and Self-Employed Parents
If you are self-employed, your child's eligibility for the Children's Health Insurance Program (CHIP) will be determined by a current 3 month profit and loss statement or a copy of your Federal Income Tax 1040 and a copy of your Schedule C.Already Enrolled in the Children's Health Insurance Program
If your child is enrolled in the Children's Health Insurance Program (CHIP) and you have billing or coverage questions, or if you would like to check the status of a new CHIP application, please call 1-877-543-7669. Unfortunately, health coverage for parents and children over the age of 19 is not available through the Children's Health Insurance Program (CHIP).Required Documents CHIP
- A copy of a current pay stub for each working parent.
- A copy of your child's birth certificate or a copy of a hospital birth record (infants only).
- If you are pregnant, a copy of a letter from a doctor or clinic showing proof of pregnancy.
- If you are receiving unemployment insurance, a copy of your award letter or your unemployment insurance check stub.
- If you are self-employed, a copy of a current three month profit and loss statement.
- If you have childcare expense, a letter or a receipt from your childcare provider.
- If you pay or receive child support, a letter or a copy of a court order.
